Main Responsibilities:

• Conduct home visits to chronic patients, elderly individuals, or those in recovery;

• Assess the patient’s clinical status and define appropriate therapeutic measures;

• Attend urgent home calls as per assigned shifts or availability;

• Record information in the electronic health record system;

• Collaborate actively with the medical and coordination teams.

• Active membership with the Official College of Physicians of Barcelona (COMB);

• Valid B-type driver’s license;

• Availability to travel daily within the province of Barcelona;

• Fluent in Spanish (mandatory);

• Knowledge of Catalan will be considered an advantage.

Preferred:

• Previous experience in home care or palliative care;

• Skills in time management, autonomy, and clinical empathy.
